The Complex Care at Home Service (CC@H) are looking for an experienced and creative physiotherapist to join their existing therapy team for six months as a secondment/fixed term contract. Our service provides proactive, preventative case management to people living with frailty and complex health and care needs. We work with adults with long term conditions that live in Gloucester, Cheltenham and the Forest of Dean.

You will be visiting people in their own homes and supporting them to live well and improve their resilience. Uniquely our focus is on what matters to them and their concerns allowing us to be truly person centred.

Our service covers 3 geographical areas of Gloucestershire, with some patients living in remote areas; so being a car driver and having access to a vehicle is essential to this role (mileage will be paid).

The post holder will use case management techniques to prevent avoidable deterioration in people’s health and ultimately reducing the occurrence of unplanned hospital admissions.

The post holder will undertake all aspects of clinical duties as an autonomous practitioner with support and guidance from the Therapy Team lead and Physiotherapy Professional Lead.

### Main duties of the job

Our team consists of Community Matrons from different professional backgrounds, Physiotherapists, Occupational Therapists, Dietician, Health and Wellbeing coordinators, Care Navigators from Adult Social Care, Frailty Housing Officer and Consultant Geriatricians. We work closely together as a multi-disciplinary team within our localities to ensure the best possible outcomes for those involved with our service. This creates a shared sense of purpose, creating a truly rewarding role. Alongside your well-honed and varied physiotherapy skills you will be utilising health coaching and motivational interviewing skills to improve a person’s health and wellbeing. The role is rewarding and challenging but never boring!

You will be supported by the Therapy team Lead, alongside your other therapy colleagues. You will have regular one-to-one Clinical Supervision from both your Lead within the service and one of the Locality Physiotherapy Leads.

The service is committed to your career progression, and we encourage and support CPD including Masters level modules and opportunities to develop within the service. Career progression can include clinical pathways such as Community Matron roles, all within the service.

### Detailed job description and main responsibilities

The post holder will:

* Provide Physiotherapy assessments and interventions to individuals requiring Physiotherapy within the Complex Care at Home service.

* Undertake all aspects of clinical duties as an autonomous practitioner with support and guidance from the Physiotherapy Professional Lead and Clinical Specialists Physiotherapists within the trust

*Provide advice and support for all members of the MDT involved in the patient's care. For example, Matrons and Wellbeing Coordinators within the CC@H service.

* Evaluate progress, re-assess and amend exercise and treatment programmes as required using specialist knowledge. To refer to other specialist services as required.
