### Job overview

An exciting opportunity has arisen to join our Community Intermediate Care Team in South East Essex. The current vacancy is a 7 month contract as a Band 6 Physiotherapist within the Cumberlege Intermediate Care Centre (CICC).

CICC is a 22 bedded Intermediate Care Rehabilitation Unit, focused on Stroke Rehabilitation. We therefore require a skilled and experienced staff member in physical rehabilitation including assessing and treating patients with stroke/neurological diagnoses.

The successful candidate will work as part of a large and dynamic MDT, comprising of Physiotherapist, Occupational Therapists, Speech and Language Therapists, Nurses, Healthcare Assistants and Therapy Assistants.

Applicants who wish to work part time will be considered.

In this team, you will have the time and opportunity to make a real difference to a patient’s functional abilities, level of independence and quality of life.

There are opportunities to develop your clinical skills, as well as developing leadership  skills to support the development of both existing and new and exciting services.

### Main duties of the job

ROLE SUMMARY

The post holder is responsible for provision of Physiotherapy assessment, treatment and evaluation of stroke/neurological patients and frailty patients in a community rehabilitation setting.

To participate in multidisciplinary rehabilitation services within a defined area of our Intermediate Care Service. The multidisciplinary team includes Rehabilitation Assistants, Therapy Assistants, Physiotherapists, Occupational Therapists, Speech and Language Therapists and Nursing colleagues

To work autonomously to provide evidence based, high quality patient care and rehabilitation which is personalised to the individual’s goals and needs.

Deliver a compassionate, dignified and respectful service to patients at all times and ensuring that the values outlined in the NHS constitution are adhered to daily and any matters of concern are raised with the relevant Line Manager or through the necessary processes within the Trust.

To work alongside colleagues in the wider Intermediate Care Service, which covers areas including:

- Cumberlege Intermediate Care Centre (Stroke Inpatient Rehab Unit)
- Early support stroke rehabilitation
- Frailty virtual ward
- Admission avoidance
- Re-ablement and unstable fracture management.

### Qualities

**Essential**

- Proven decision making ability and problem solving skills
- Enthusiastic and able to work without direction

### Skills & Experience

**Essential**

- Previous experience as physiotherapist working in NHS or similar UK health systems
- Experience of working in a range of clinical areas e.g. Stroke, orthopaedics, neuro, medical
- Ability to prioritise and delegate workload and own time

**Desirable**

- Experience working in and leading a multi-disciplinary rehabilitation team
- Experience of working in a community environment
- Experience and knowledge of using SystmOne
- Neurological/ stroke assessment and rehabilitation skills

### Education /Qualifications

**Essential**

- BSc or Diploma in Physiotherapy and member of the Chartered Society of Physiotherapists (CSP)
- HCPC Registration

**Desirable**

- Evidence of Continual Professional Development (CPD)
- Postgraduate study in relevant area e.g. Stroke, Falls, Frailty
