Working closely with Clinical Pharmacists, nursing staff and the wider multidisciplinary team, you will play a key role in medicines reconciliation, discharge planning, patient counselling and improving patient outcomes through high-quality medicines optimisation services.The successful candidate must be registered with the GPhC and committed to continuing professional development.
