This is a full time post. ### Main duties of the job You will need to be a confident decision maker, with clear leadership skills, excellent pharmaceutical knowledge and technical skills and a sound understanding of pharmacy service and hospital service. Main duties include: Lead, deliver, develop, and assess the value of clinical pharmacy services to support Hospital at Home service. Support the development of good practice and safe medicines management and optimisation systems across Herefordshire and Worcestershire Integrated Care System (ICS). Work in collaboration with different organisations across the ICS to provide highly effective clinical pharmacy services to patients within Hospital at Home service. Develop and work within appropriate governance structures for the designated clinical service liaising with ICS wide stakeholders as needed.
