We have a rare opportunity for permanent position to lead and oversee clinical pharmacy input to the Urgent Community Response (UCR) and frailty virtual ward team to deliver care to the virtual ward and Urgent Care Response teams. You will contribute to patient care both within the unit base setting, and via patient home visits where appropriate. Lead on medication provision and delivery across organisational boundaries. Complete Medication reviews, medicines reconciliation and pharmaceutical care plans for patients admitted to the virtual ward. Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role. ### Main duties of the job You will work as an independent prescriber, optimising medicines and promoting appropriate, evidence-based, and cost-effective prescribing and deprescribing as part of the wider hospital at home team.
