### Job summary

This is a fantastic opportunity for an experienced OT to join Droitwich, Ombersley & Rurals Neighbourhood Team. Were a friendly, supportive and hardworking team delivering compassionate community care.

This is a full time post.

You will be integrated with nurses and therapists and instrumental in the journey of patients on the intensive support pathway. Assessing patients being discharged from hospital or those requiring support to prevent admission, identifying short-term care support for rehabilitation and immediate equipment needs. The role is fast paced and responsive.

Alongside intensive support, you will triage and assess patients requiring functional OT assessments in their homes, including manual handling and provision of specialised and standard equip, working closely with GPs, physios, social services, HASE (housing adaptations & specialist equip) and the voluntary sector.

There are opportunities for training and development, alongside in-service training, appraisals and clinical supervision.

### Main duties of the job

A shift is 7.5hrs between 0830 1630hrs, Mon to Fri.

A typical day includes coordinating your caseload, triaging referrals, supporting junior staff and assistants and prioritising urgent assessments alongside colleagues.

The role will involve:

Completing holistic assessments of patients needs following hospital discharge or to prevent admission.

Completing assessments for low level equipment including riser recliner chairs, minor adaptations (e.g. rails, half steps) and manual handling.

Co-ordinating care needs of patients on the intensive support team by liaising the GP, MDT, relatives and other agencies. Including organising individual client case reviews, communicating plans with the MDT, and being responsible for the ongoing actions in relation to the patient.

Line management of Band 5 and Band 3 team members including training, supervision and appraisals.

Maintaining contemporaneous clinical notes to the high professional standards of the COT.

### Knowledge

- A range of evidence based Occupational Therapy clinical skills and knowledge appropriate to specialism.
- An understanding of the approaches and models of practice relevant to practice.
- A good level of knowledge in a wide range of physical and mental health conditions.
- Good level of knowledge relating to disability, equipment and adaptations.

**Desirable**

- A working knowledge of Occupational Therapy Outcome Measures and Models of Practice.
- Working knowledge and experience of Occupational Therapy standardised assessments appropriate to the role.

- A range of post registration experience including in a health and social care setting.
