The post holder will be responsible for assessing, planning and delivering occupational therapy interventions for patients with a wide range of clinical needs and conditions, including orthopaedic, neurological, medical and surgical presentations, as well as frail older adults with complex co-morbidities. A key aspect of the role will involve supporting patients to maximise their independence, facilitating safe discharge planning, and working closely with patients, families and carers to achieve personalised rehabilitation goals. The post holder will work as part of a large multidisciplinary team committed to delivering and continually improving high standards of therapy within Dudley Group NHS Foundation Trust. The post may close early if a significant number of applications are received. ### Main duties of the job To work as part of the intermediate care pathway, providing effective and efficient physiotherapy to patients with a variety of physical conditions as part of an multi-disciplinary team. Duties will include, assessment, therapy (both group and/or individual as appropriate), developing tailored therapy programmes through to discharge, as well as appropriate delegation of work to assistants. Working autonomously across various ICT units, using evidence based/client centred principals to assess, plan, treat and evaluate interventions.
