This is an exciting opportunity to be part of an innovative, fast-paced team delivering hospital-level care in patients' own homes. You will play a key role in providing rapid, holistic assessments and interventions for adults with acute illness, frailty, and exacerbations of long-term conditions. Working autonomously, you will deliver specialist occupational therapy to support admission avoidance, early discharge, and the promotion of independence and quality of life. As part of a highly skilled multidisciplinary team, you will work closely with Consultant Practitioners, ACPs, GPs, community services, ambulance crews and social care partners to provide coordinated, person-centred care. You will contribute to complex case management and lead therapy input, bringing your clinical expertise to shape patient care. You will also support the development of others through supervision, teaching and role modelling best practice. Working within a responsive and supportive team, you will demonstrate flexibility and teamwork in meeting the needs of a rapidly changing caseload, ensuring timely and compassionate care for patients.
