You will assess functional ability, manage risk, support recovery-focused goals, and work closely with families and community services to maximise independence and ensure safe and timely discharge from hospital. You will have opportunities to influence and shape service delivery through quality improvement and service development initiatives, helping to enhance patient experience and clinical effectiveness. As a senior member of the team, you will provide supervision, support and mentorship to junior colleagues, students and support staff, and will deputise for the Team Lead when required.
