This post is based within Hawthorn Recovery Unit, which provides assessment, treatment and rehabilitation for older adults with functional mental health problems. The unit plays a key role in admission prevention for those with acute and complex mental health needs.

### Main duties of the job

The post holder will hold an allocated caseload acting as named Occupational Therapist and Care Coordinator as designated, formulating care plans and risk management plans with the involvement of the client and family members.

The post holder will be responsible for delivering assessment and treatment sessions and for contributing to multidisciplinary care planning, risk assessment and discharge co-ordination.

The postholder will supervise staff and students as required, and support induction and development of staff and students within the team.

The post includes taking the in-charge role for the team for designated shifts.

### Detailed job description and main responsibilities

As a Band 6 Occupational Therapist and Care Coordinator within the Hawthorn Recovery Unit, you will provide specialist assessment, intervention and care coordination for older adults with complex mental health needs. You will manage a designated caseload, undertaking comprehensive occupational therapy assessments, including functional and cognitive assessments, and developing person-centred care plans that promote recovery, independence and wellbeing. You will deliver and evaluate individual and group interventions, support risk management and discharge planning, and work closely with service users, carers and partner agencies to ensure coordinated and effective care.

You will play an active role in the day-to-day operation of the unit, maintaining high standards of clinical documentation, participating in care reviews and contributing to service development initiatives. You will be responsible for managing your workload effectively, responding flexibly to changing clinical priorities and supporting the delivery of safe, high-quality care in line with Trust policies and professional standards.

As a senior member of the team, you will provide clinical leadership, take charge of designated shifts, supervise and support staff and students, and contribute to training and professional development within the service. You will be expected to lead on specific areas of practice, support service improvement projects, maintain your own continuing professional development, and uphold the standards of the Health and Care Professions Council and the Royal College of Occupational Therapists.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

- Ability to appreciate and apply continuous improvement strategies in practice.
- Specialist knowledge of statutory policy relating to mental health care delivery incl. Dialog plus, Mental Health Act and Mental Capacity Act

- Motivated to learn to enhance knowledge and continuously develop skills.
- Special interest in working with carers

### Skills/Abilities

- Ability to undertake comprehensive risk assessment and implement risk management plans.

### Experience/Knowledge

**Essential**

- Post registration experience as a Band 5 OT preferably in a mental health setting working with older adults with functional and organic mental health needs
- Knowledge/ Experience of Safeguarding Adult Processes
- Experience of developing care plans with individuals and carers

**Desirable**

- Experience of working as a care coordinator in mental health.
- Experience of carrying out assessments independently including risk assessment and mental capacity assessment
- Experience of using Dialog + and co-producing care plans

### Qualifications/Registrations

**Essential**

- Dip/BSc (Hons) in Occupational Therapy
- Evidence of further post-registration education and training
- Currently Registered with appropriate professional body
- Evidence of ongoing CPD
- Post registration experience as an Occupational Therapist, preferably in a mental health setting working with older adults with functional and organic mental health needs.

**Desirable**

- Further training within the specialist area of Mental Health for the Older Person.
- Ability to demonstrate transferable skills if currently working in another setting.
- Recognised training in teaching/ mentorship/ supervision.
- Experience of carrying out assessments independently including risk assessment and mental capacity assessment
- Experience in working with older adults.
- Experience of group and individual work
