### Job overview

The post holder will deliver an expert healthcare service to adults within HMP Aylesbury. They will offer advice to clinicians, allied healthcare professionals.

They will provide a comprehensive primary care physical health service and work primarily within primary care which is part of an integrated healthcare service which includes Primary care, wellbeing (which includes mental health and substance misuse).

To undertake physical health assessments, undertake clinics, provide and evaluate a range of health treatments and interventions, to meet the patient's healthcare needs and contribute to the wider continuous development of healthcare services.

Demonstrate knowledge, skills and understanding of health issues and treatment options to provide physical health clinical guidance to all staff on matters relating to physical health and wellbeing.

To ensure high standards of primary care services are delivered in a changing healthcare environment, ensuring that the needs of the patients are at the centre of practice.

The post holder is expected to continually promote high standards of service within the nursing team and contribute to development of a culture of professionalism within the team.

### Main duties of the job

To plan, deliver and evaluate interventions for treatment as clinically appropriate using evidence-based models.

To set up processes/protocols for effective assessment, planning, implementation and evaluation of care for those patients with physical and mental health and substance misuse problems.

To safeguard vulnerable patients in line with Trust safeguarding policy.

To apply local Risk Assessment and management protocols.

To make appropriate referral to other available services for longer term interventions.

Co-ordinate multi-disciplinary clinical practice to provide focused and integrated care and treatment programmes/packages.

To be aware of the appropriate handling of aggressive patients and the appropriate procedures for dealing with them.

Work collaboratively with staff of other disciplines e.g. officers and governors to provide integrated and coordinated care.

To work within own competency and ensure that any training needs are communicated to the team manager.

To participate in local audits and trust audits to enhance quality of care. To participate in clinical governance and utilise research skills for audit, service development and research within the area served by the multi-disciplinary team.

### Detailed job description and main responsibilities

To liaise with other professional services

To work in a highly emotive atmosphere with clients with offending histories and / or mental health problems on a regular basis, and be able to communicate sensitive and complex information, whilst maintaining a high degree of professionalism at all times.

To ensure that efficient pathways of care are being created for the trainees, including collecting collateral information for new trainees and making appropriate referrals into the community when they are approaching discharge.

To work closely with colleagues to ensure information in relation to risk is collected and communicated sensitively and comprehensively.

To maintain the highest standards of clinical record keeping, including electronic data entry and recording.

To ensure that with all aspects of professional activity (e.g. communication with patients, relatives, colleagues as well as in the assessment, development/delivery of interventions) issues of diversity such as race, culture, identity, gender and sexuality are sensitively considered and incorporated to reflect individual differences appropriately.

### PREVIOUS EXPERIENCE

**Essential**

- - Experience at senior band 5 grade
- - Experience in providing and receiving supervision
- - Experience of running clinics and/or group work.
- - Ability to keep accurate records.
- - Practical experience of providing high quality care and exceeding expectations

**Desirable**

- - Experience of working within a secure environment
- - Experience of systmone IT system

### EDUCATION AND QUALIFICATIONS

**Essential**

- - Registered General Nurse
- - Evidence of personal and professional development in Nursing since Registration

**Desirable**

- - Post registration qualification appropriate to speciality
- - ENB 998 or equivalent
- - Chronic disease management or long term condition qualification

### SKILLS, KNOWLEDGE, ABILITIES

- - Knowledge of medication rounds and the safe storage and administration of drugs.
- - Knowledge of, and ability to follow, NMC code of conduct
- - Demonstrates the ability to manage work under pressure
- - Ability to assess plans, implement and evaluate patient care & act accordingly if untoward changes occurred.
- - Knowledge of risk assessment and management and its implementation.
- - Ability to treat patients with respect and dignity.
- - Ability to use computer to access patient information and use email.
- - Able to use computer software packages such as MS Word and Excel
- - Sensitive to colleagues and able to promote a positive motivating environment.

- - Ability to maintain patient records within standards required by locally, Trust & NMC
