The Enfield Integrated Proactive Care Service delivers care through a multidisciplinary, multi-agency care coordination model that underpins neighbourhood-based care. Bringing together professionals from health, social care and the voluntary and community sector, the service supports adults living with frailty and complex needs. It provides a strong foundation for the continued development of Integrated Neighbourhood Teams, demonstrating the benefits of truly joined-up, preventative and person-centred care delivered within local communities.

### Main duties of the job

The Senior Mental Health Clinical Nurse Specialist will work as part of the Integrated Proactive Care Service multidisciplinary team, providing specialist mental health support to adults with complex physical and mental health needs living in the community. The post holder will undertake comprehensive mental health assessments, develop and implement personalised care plans, provide expert clinical advice, and support patients to improve their health, wellbeing, and independence.

The successful candidate will work closely with GPs, community services, social care, mental health services, hospital teams, and voluntary sector partners to coordinate care, reduce health inequalities, prevent avoidable hospital admissions, and support people to receive care closer to home. They will provide specialist advice on mental health pathways, risk management, safeguarding, medication adherence, and complex case management.
