We are looking for a motivated individual with a passion for improving patient care and patient experience. MICH: The Milford Integrated Care Hub is run as outpatient department and provides diagnostic process to determine the medical, psychological and functional capability of someone living with frailty. By providing holistic assessment for older people with complex needs, aim is to improve the patient’s quality of life, promote self-care, reduce risk of deterioration in patient’s condition and avoid unnecessary hospital admission. ### Main duties of the job The Band 3 Health Care Support Worker will carry out delegated tasks to support the delivery of high-quality patient care within the Milford Integrated Care Hub.
