### Job overview

Post is for a qualified learning disabilities nurse to work within the mental health inreach team.

Work within a busy /challenging environment with offenders with learning and mental health needs.

Work alongside wider MDT to ensure safe care and management of prisoners at HMP Manchester

### Main duties of the job

Support individuals to understand and engage with healthcare interventions, prison regimes, and legal processes.

- Monitor physical and mental health needs and coordinate appropriate referrals.

- Ensure prisoners receive appropriate reasonable adjustments in line with the Equality Act.
- Advocate for vulnerable prisoners to reduce health inequalities and improve access to services.
- Assist individuals in understanding information through accessible communication methods.

Work collaboratively with mental health teams, primary care, prison staff, psychologists, probation services, and external agencies.

- Participate in multidisciplinary meetings and case reviews.
- Contribute to sentence planning, risk management, and discharge/release planning where appropriate.
