We are seeking an exceptional and experienced healthcare professional to join our Continuing Healthcare Service as a Senior Complex Care Practitioner. This is an exciting opportunity for an ambitious leader with extensive NHS Continuing Healthcare experience to play a pivotal role in shaping and delivering high-quality, equitable and sustainable complex care services across North Central London.

As a senior clinical and operational leader within the Complex Care Service, you will provide expert oversight of highly complex cases, support strategic service development and lead a team of skilled practitioners delivering NHS Continuing Healthcare services. Working across organisational boundaries, you will collaborate with local authorities, provider organisations, acute and community services, primary care and wider system partners to improve patient outcomes, support discharge pathways and ensure services deliver value for money whilst maintaining the highest standards of quality and safety.

This role offers a unique opportunity to influence the future direction of Continuing Healthcare services, contribute to system-wide transformation programmes and support the delivery of integrated care across North Central London.

### Main duties of the job

- Provide senior clinical leadership and expert advice across all aspects of NHS Continuing Healthcare.
- Oversee the management of highly complex cases, including appeals, Local Resolution Meetings, Independent Review Panels and retrospective reviews.
- Lead, develop and support a multidisciplinary team, fostering a culture of continuous improvement, inclusion and compassionate leadership.
- Manage service performance, workforce planning and operational delivery to ensure key targets and quality standards are achieved.
- Contribute to the development and implementation of service improvement initiatives, pathways and transformation programmes.
- Work collaboratively with system partners to improve patient flow, discharge pathways and integrated care delivery.
- Support budget management and ensure care packages deliver high-quality outcomes and value for money.
- Lead on governance, quality assurance, risk management and compliance with relevant statutory and regulatory frameworks.
- Represent the service at senior-level meetings and contribute to strategic planning across the wider health and care system.
- Promote equitable, patient-centred services that improve outcomes and reduce health inequalities for patients, families and carers.

### Experience

**Essential**

- Evidence of leadership development activities
- Evidence of continuing professional development
- Evidence of managing a team
- Evidence of managing a budget
- Significant experience in all aspects of CHC, including appeals, LRM, IRP’s retrospectives
- Significant experience of managing and supporting others with complex cases, and legal aspects pertaining to these types of cases.
- Experience of working with health and social care systems to deliver effective pathways of care

**Desirable**

- Previous involvement with efficiency and cost improvement programme

### Education/Qualification

- Educated to Masters or equivalent level of experience (see guidance document further details).
- Current registration with professional body e.g. NMC and hold a clinical qualification e.g. Registered Nurse, Registered Learning Disability Nurse or Registered Mental Health Nurse, AHP
- Membership of a professional body
