The Urgent Out of Hospital Service is seeking an experienced Senior Nurse to join our innovative and expanding team. Bringing together Hospital@Home, Urgent Community Response (UCR) and Virtual Ward services, we are at the forefront of delivering integrated care closer to home, supporting the NHS vision of providing high-quality, patient-centred care outside of hospital settings. Based at Milford Hospital, our team receives referrals from a wide range of partners across the Guildford and Waverley community, including GP practices, ambulance services, social care teams and voluntary sector organisations.
