An exciting opportunity has arisen for the recruitment of a Senior community nurse within the District Nursing night team. This appealing role will enable the successful person to support the leadership and management of staff and the caseload within the District Nurse night service to meet the needs of the population.

Interested applicants will need to be registered nurses who have relevant nursing qualifications to degree level, specialist post registration education and leadership training or equivalent experience.

### Main duties of the job

We are looking for the successful candidate to be able to:

- assess, plan, implement and evaluate care for a defined caseload
- ensure that the care needs of the caseload are met by effective management and appropriate delegation of work to other members of the team
- work in partnership with the primary health care team and liaise and cooperate with everyone involved in holistic patient care.
- be able to demonstrate extensive knowledge, advanced analytical and practical skills, and the ability to communicate in a non-judgemental, empathetic way with patients, carers and other professionals.
- Provide clinical supervision and leadership to junior staff and support staff development

The post holder will need to have good communication skills both written and verbal, have evidence of ongoing professional development and be able to work independently.

### Detailed job description and main responsibilities

Clinical care includes

Assess needs, plan, implement and evaluate programmes of care for individuals and families over a 24 hour period, involving the patient, family and other carers at all stages of care and taking into account complex care needs.

Undertake and be competent in complex clinical skills and case management to meet the needs of the patients on the caseload.

Provide palliative and terminal care to patients and support and advice to carers.

Verify expected deaths and complete the relevant documentation.

Practice within current guidelines for the management of medication.

Manage the polypharmacy and other medication issues with liaison with GPs as required.

Provide specialist clinical advice based on evidence to members of the team and others involved in holistic patient care.

Liaise with other health and social care professionals caring for patients in the community.

Support the avoidance of unnecessary hospital admissions and also liaise with secondary care teams in order to facilitate continuity of care for patients, facilitating early discharge and evaluating the process.

## Person Specification

### EXPERIENCE

**Essential**

- Minimum 2 years community nursing experience
- Up-to-date specialist clinical knowledge
- Experience in the management of complex/long-term patient care
- Evidence of ability to direct and co-ordinate programmes of care working autonomously
- Experience in working collaboratively with other disciplines/partners
- Experience of managing a team

**Desirable**

- Advanced clinical skills

### QUALIFICATIONS

**Essential**

- Registered General Nurse with relevant qualification at degree level
- Community Specialist Practitioner Qualification
- Evidence of continuing Professional Development
- V300 Nurse Prescriber or willing to undertake

**Desirable**

- Leadership training or equivalent experience
- Minimum 2 years experience in leadership role

### SKILLS / ATTRIBUTES

**Essential**

- Ability to identify high risk patients and plan and implement care
- Excellent leadership skills
- Knowledge of current NHS and Social Care systems and drivers
- Car owner/driver with full UK driving license
