Our Hub and intervention community mental health team provides timely person cantered support to adults experiencing moderate to severe mental health difficulties . Working as part of an integrated multidisciplinary service, we focus on early intervention recovery and adopt a person centred approach. There are varied opportunities for training and development, and peer support and supervision systems are in place. Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the time of applying. This role may be eligible for sponsorship under the UK Visas & Immigration (UKVI) Skilled Worker route. Undertaking Comprehensive mental health assessments and risk assessments Providing short to medium term interventions and care coordination managing a defined caseload within the community Working closely with psychiatrists, psychologists, social workers and voluntary sector partners Supporting crisis prevention and recovery focused care planning Contribute to triage and decision making within the hub. To be compassionate in meeting the needs of patients and their carers. To work as part of an integrated community team, working collaboratively and in partnership with local authority services. To be professionally accountable and responsible for patient care undertaking a range of clinical/therapeutic interventions in patients’ homes and other community settings. To promote social inclusion, community access and participation through the provision of patient care. To facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and clinical supervision to associate practitioners, community support workers and students as appropriate. To promote at all times a positive image of people with mental ill health.
