An exciting opportunity has arisen for x 1 Band 6 Registered Mental Health Nurses to join East Durham Community intervention and Hub Mental Health Team.

Our Hub and intervention community mental health team provides timely person cantered support to adults experiencing moderate to severe mental health difficulties . Working as part of an integrated multidisciplinary service, we focus on early intervention recovery and adopt a person centred approach.

There are varied opportunities for training and development, and peer support and supervision systems are in place.

Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the time of applying.

This role may be eligible for sponsorship under the UK Visas & Immigration (UKVI) Skilled Worker route. ### Main duties of the job

To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and well-being.

Undertaking Comprehensive mental health assessments and risk assessments

Providing short to medium term interventions and care coordination managing a defined caseload within the community

Working closely with psychiatrists, psychologists, social workers and voluntary sector partners

Supporting crisis prevention and recovery focused care planning

Contribute to triage and decision making within the hub.

To be compassionate in meeting the needs of patients and their carers.

To work as part of an integrated community team, working collaboratively and in partnership with local authority services.

To be professionally accountable and responsible for patient care undertaking a range of clinical/therapeutic interventions in patients’ homes and other community settings.

To promote social inclusion, community access and participation through the provision of patient care.

To facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and clinical supervision to associate practitioners, community support workers and students as appropriate.

To promote at all times a positive image of people with mental ill health.

To promote at all times a positive image of the Adult Mental Health Service and the wider Trust.

## Person Specification

### Knowledge

**Essential**

- Demonstrable knowledge to post-graduate level of evidence- based practice in caring for patients in the designated field
- Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
- Detailed understanding of Safeguarding and its application in practice.
- Clinical Risk Assessment and Management and its application in practice.
- Clinical Governance and its application in practice.
- Research and development methodology.
- Understanding of psychological models of care and treatment.
- Working knowledge of clinical medicines management, including administration of injections

### Experience

**Essential**

- 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework)

- Providing clinical supervision to individuals or groups as professionally appropriate.

### Qualifications

**Essential**

- Current professional registration with Nursing & Midwifery Council - RNMH or RNLD (RNSC – CAMHS only ) – RGN (If appropriate)
- Evidence of Continuing Professional Development.
- Recognised Practice Supervisor and/or Practice Assessor, clinical educator or equivalent. Must be achieved within 6 months
- Trained in Clinical Supervision and providing clinical supervision OR willingness to work towards within 6 months
