If successful, you will ensure a high-quality service is provided and care is delivered to all service users on the HTT and RR caseload who are experiencing a Mental Health Crisis, by the Trust’s policies and procedures and the care programme approach. The post holder must hold a full, valid UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010). ### Main duties of the job - Contribute to the implementation of clinical and interdisciplinary strategies to ensure the delivery of a high standard of care and service objectives and to the overall corporate objectives of the organisation. - Work as a member of the Crisis Home Treatment Team, ensuring team clinical performance is optimised, meeting agreed KPIs and ensuring that CRHT is a responsive service providing timely assessment and offering a range of home treatment interventions as an alternative to hospital admission. - Have robust working relationships with colleagues within the acute and community settings; liaison psychiatry, place of safety, and inpatient teams, so seamless acute care is provided for service users. ### Detailed job description and main responsibilities The post holder will work in collaboration with colleagues from the community recovery care group, the local authority, primary care and other key stakeholders to ensure effective care delivery and timely onward care at the point of discharge At Kent and Medway Mental Health NHS Trust, we are serious about diversity and inclusion, and we are working hard to build this into our DNA.
