The post holder will work in an integrated work across the Mary Seacole Unit and the @home virtual ward. The post holder will support our goal in improving the quality of care delivered to patients with a focus on maximising peoples’ independence and supporting their return to their usual place of residence wherever possible. You will receive peer support from fellow doctors working across the frailty services in Epsom General Hospital and Surrey Downs Health and Care Partnership and be fully supported consultant geriatricians and an integrated management team helping you to drive forward change. ### Main duties of the job - To show dynamism, enthusiasm and flexibility in participating in the supporting and shaping of community and interface frailty services as part of SDHC. - To help provide a first class clinical service to all patients. - To provide high quality leadership and support to medical staff, nursing and allied health care professionals across the Homefirst Service. - To lead and support the multi-disciplinary team with complex discharge planning. - To support and sustain teaching and research where possible in relevant clinical areas. - When working in the community, the successful applicant will be expected to bring core medical skills; expertise and leadership to a wider team of clinicians tasked with the delivery of multidisciplinary assessment and integrated multi-agency interaction. - To work within the framework of applicable Trust policies, procedures, and clinical governance framework. - To work within contractual requirements, and in support of, applicable Trust clinical development frameworks, business plans, and development strategies. ### Detailed job description and main responsibilities The principal duty of this post is to provide a comprehensive clinical service to patients. - To at all times, comply with the GMC’s guidance on ‘Good Medical Practice’ as amended or substituted from time to time. - To ensure that patient confidentiality is maintained at all times. - To undertake duties as defined within the attached job plan in so far as practicable and in agreement with your Clinical Director. - To be responsible and managerially accountable for the reasonable and effective use of Trust resources that you use and influence, and professionally accountable to the Clinical Director. ## Person Specification ### Experience **Essential** - Evidence of continuing professional development ### Qualifications **Essential** - MBBS or equivalent - Has completed four years of training with two years in specialties relevant to geriatric medicine ### Personal Attributions **Essential** - Adaptability, flexibility and ability to cope with uncertainty - Willing to engage with and learn from peers, other professionals and colleagues in the desire to provide or support the most appropriate interventions - Professional, calm and efficient - Effective organizer/prioritisation skills **Desirable** - Highly motivated with ability to influence and inspire others ### Skills & Capabilities **Essential** - Evidence of Team working skills - Evidence of excellent communication skills - Ability to make decisions autonomously, when required, but understanding when to ask for help - IT Skills, Working knowledge of Microsoft Office with intermediate keyboard skills - Equality and Diversity Needs to have a thorough understanding of and commitment to equality of opportunity and good working relationships both in terms of day-to-day working practices **Desirable** - Project Management skills (prioritising, planning, adapting plans, resource planning) - Management Skills and leadership skills - Must be able to prioritise own work effectively and be able to direct activities of others.
