In collaboration with University Hospital Southampton NHS trust (UHS), the service works closely with the medical team at the hospital and GPs in the community to facilitate acute assessment with the aim to prevent & minimise admissions to hospital and facilitate early discharge where possible. There's lots of training available to gain new and enhance existing skills and will be development opportunities available. Community Staff Nurse The service working hours at 8am until 8pm 7 days a week. ### Main duties of the job The Children's Hospital @ Home team provide specialist nursing care, support, and co-ordination of care; predominantly to children and young people with short term health care needs requiring urgent care pathways enabling early, safe planned discharge from hospital and prevent unnecessary admissions.. Manage a caseload and carry out specific health care assessments and/or interventions. To carry out initial assessment of patients with short term health needs by telephone. Provide remote clinical assessment via telephone or video call. Autonomous clinical assessment of acutely unwell child in the home environment Provide supervision and monitoring of junior staff with support of clinical team leader. Participate in the administration of medicines including intravenous therapy, in partnership with pharmacy colleagues. Working collaboratively with all professionals to enable children/young people to be cared for at home whenever possible. The information received is often of a complex and sensitive nature requiring sensitivity, empathy and reassurance. Demonstrate excellence in clinical practice, keeping up-to-date with new developments in nursing care and treatments ensuring practice is evidence based by utilizing current research.
