An exciting opportunity has arisen for a full-time Senior Care Act assessor within a well established and dynamic initial response team.

The position is within the older persons with physical disability team which is a community-based team working with Adults over 18 years of age with a range of physical and Mental health needs. The successful applicant will hold a level 3 diploma in adult care or lead adult care/ social care worker and or level 4 diploma in adult care or lead practitioners in adult care level or be willing to complete/ achieve this if a level 3 qualification is already held. The successful candidate will also have experience working within a relevant field.

This is a dynamic post that requires the post holder to work closely with multi-disciplinary teams/agencies in addressing a range of care/support issues in the promotion of independence and improving the health and wellbeing of the population served.

### Main duties of the job

The post holder will be expected to access Social care and health records to gather information, therefore it is important for the post holder to be able to use a computer and have an understanding of the importance of good recording. You will require effective communication and written skills and be willing to work as part of a team to promote best outcomes for individuals. You will be required to be self-motivated and organised in order to manage any competing demands and be provided with all relevant training and support in the role.

Midlands Partnership Foundation Trust is dedicated to ongoing Social Care development, and you will have access to the Social Work learning academy in order to promote professional development. You will also receive the support of your team and your line manager who will provide regular supervision both on a formal and informal basis.

Social Work Learning Academy (SWLA)

On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA ‘stage’ (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

### Applicant requirements

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

## Person Specification

### Experience

**Essential**

- The ability to demonstrate a commitment to working in partnership with adults, carers, professionals and voluntary agencies, to provide a comprehensive service which meets the needs of the adults in line with current legislation and directorate, local authority & Trust policies
- Experience of working in a team, providing formal support and supervision to junior colleagues, incorporating good interpersonal relationships and the ability to actively contribute to the implementation, development and review of an individual’s care and support plan
- Experience of working within the specialist area
- Experience of working in an NHS/Local Authority setting where MDT and specialised practice associated to social work and social care is required
- Experience in dealing with complex casework involving family members, carers, communities and other organisations

**Desirable**

- Experience of providing informal and formal staff supervision

### QUALIFICATIONS & TRAINING

**Essential**

- Level 3 Diploma in Adult Care or Lead Adult Care / Social Care Worker
- Level 4 Diploma in Adult Care or Lead Practitioners in Adult Care Level or willing to complete / achieve this if a Level 3 qualification is already held
