### JOB SUMMARY

The post holder will work as a dedicated senior occupational therapy member of staff within the Community Rehabilitation Team (Intermediate Care) using practical skills to provide high quality & specialist occupational therapy assessment and interventions to patients referred to the service.

The focus of this post will be on helping clients referred to return to previous levels of functioning or become as independent as possible in the everyday occupations they need and like to do, including self-care, leisure and productivity. In doing so, restoring/improving occupational performance in individuals either temporarily or permanently disabled due to illness or injury in order to assist them to live as independent as possible within their own home/living environment. The post holder will have a clinical role as well as supervisory/management responsibilities for staff including Occupational Therapists and Rehabilitation Assistants workers ensuring that all external and internal quality standards are achieved and the service meets all the agreed performance indicators.

The post holder will work within a multidisciplinary team, responding to referrals from acute and community settings. The post holder will assess and review needs, optimising independence, make recommendations upon the outcome or review regarding social care needs and appropriate onward referrals to relevant Teams.

### KEY DUTIES / RESPONSIBILITIES

Clinical/Professional/Specific Managerial Responsibilities

To be professionally and legally accountable for all aspects of own clinical and professional work, including the management of patients/clients in your care. Ensure that your own practice and that of staff under your supervision meet the Health and Care Professions Council (HCPC) Occupational Therapy Standards of Proficiency; Standards of Conduct, Performance and Ethics and Royal College of Occupational Therapist’s Practice Guidelines and meets national and local and Trust standards of practice as required.

To autonomously manage a caseload of routine and complex patients requiring specialist occupational therapy assessment and interventions to ensure rehabilitation is of the highest and evidence-based standards with consideration given to all treatment options.

Prioritise and plan own workload to manage conflicting and competing priorities both clinically and professionally, to ensure patient care & treatment is carried out in an efficient and timely manner.

To undertake comprehensive standardised and non-standardised assessments of clients with a range of health & social care needs and multi-pathologies; use advanced clinical reasoning, analytical and communication skills to determine diagnosis and prognosis of condition. This will include an advanced assessment of patients’ physical, cognitive, psychological, social and environmental levels of functioning and will require a sound knowledge of normal movement and occupational performance.

Formulate and implement occupational therapy treatment plans based on current evidence and a sound knowledge of available treatment options, using advanced clinical assessment and reasoning skills and a knowledge and ability to use different treatment modalities i.e manual techniques (range of movement assessment, therapeutic handling and positioning), skills development/rehab and compensatory techniques or prescription of medical devices, task analysis, patient education and splinting.

To take a lead role within the multi-professional team for the safe and efficient discharge of patients from the service, including liaison and negotiation with a range of service providers (statutory & voluntary), specialist assessment of client’s proposed home/living environment, including social supports and house design/ layout. This requires sound knowledge of a range of health & social care policies and eligibility criteria.

To initiate and undertake risk assessments when assessing and treating patients. This will involve an assessment of the individual and the environment and include advising the multi-professional team on appropriate management, specifically relating to moving and handling, safe use of medical devices and home risk assessments.

To continuously promote and participate in service improvements through quality improvement methodology, supervision, audit and other CPD activities, as appropriate to the needs of the Community Rehabilitation Team. Make recommendations for change to the Service Lead / Clinical Lead Occupational Therapist.

To carry out risk assessments and implement and review risk management strategies in complex cases in relation to Manual Handling in areas of personal and domestic activities of daily living and the environment.

To maintain a CPD portfolio in accordance with HPC requirements and monitor own and those you supervise, professional standards in accordance with the College of Occupational Therapists Code of Ethics & Professional Conduct, Professional Standards for Practice and the HPC Standards of Proficiency for Occupational Therapists.

Generic managerial responsibilities both within an operational and strategic

Assist and support the professional development and competency of those staff you supervise including participating in the staff appraisal process as a reviewer

To assist in monitoring occupational therapy resources, identifying and requesting new/replenished resources as necessary by informing the Team Lead for Occupational Therapy within the Community Rehabilitation Team.

Undertake the collection of data for statutory reporting, audit and research projects.

To have responsibility for the health, safety and welfare of self and those you supervise and to comply at all times with the requirements of health & safety regulations.

To attend mandatory induction and training courses as designated by the Trust.

To abide by regulatory body (HCPC) and professional body (RCOT) Standards of Proficiency, Conduct, Performance & Practice at all times.

Take responsibility for his/her own ongoing learning and development, including full participation in appraisals, in order to maximise his/her potential and continue to meet the demands of the post.

### OPERATIONAL DELIVERY

Contribute to the review of the Community Rehabilitation Service to identify opportunities for improvements and implement efficient and cost effective working practices.

Contribute to the collation of service user/carer feedback.

Be responsible for maintaining accurate and comprehensive service user intervention records in line with RCOT, HCPC & local standards of practice.

Supervise more junior staff, and Rehabilitation Assistants to ensure their compliance with local, professional and regulatory body service standards.

To be responsible for the safe and competent use of all specialised equipment, patient appliances and aids by service users. To ensure that more junior staff members, and support staff attain competency prior to use.

Be responsible for maintaining own competency to practice through CPD activities, and maintaining a portfolio, which reflects personal development (Life Long Learning).

Maintain and further develop an up to date knowledge of evidence-based practice in relation to Intermediate Care Services.

Be active in supporting training by attendance and participation in, in-service training programmes, tutorials, individual training sessions, external courses and peer review.

Be responsible for auditing and evaluating own practice and the practice of other staff through supervision processes.

Take such action as may be necessary in disciplinary matters in accordance with procedures laid down by the Trust.

Review individually, at least annually, the performance of immediately subordinate staff, provides guidance on personal development requirements and advises on and initiates, where appropriate, further training.

Delegate appropriate responsibility and authority to the level of staff within his/her control consistent with effective decision making, while retaining overall responsibility and accountability for results.

Promote and maintain a high quality service, which demonstrates a sound knowledge of Clinical Governance and Risk Management, as it applies to the work situation.

Be actively involved in professional clinical groups, Peer Review Groups and other professional development activities.

Input to the development/review and potential change, of all relevant protocols and policies. Many of these policies have Trust wide implications across a range of professions/patient groups. Assist in the supervision and co-ordination of more junior staff, students and Rehabilitation Assistants on a daily basis and participate in the induction of new staff.

Be responsible for the safe and competent use of patient appliances, by patients, other peers, junior and student Occupational Therapists, through teaching, training and supervision of practice.

Perform any other duties as may be requested by the your line manager.

Maintain staff relationships and morale amongst the staff.

Participate, as required, in the selection and appointment of staff in accordance with procedures laid down by the Trust.
