Job Summary As part of The Chislehurst Partnership team, you will provide NHS primary health care to patients registered with the practice. This will also include a share of the Duty doctor rota, paperwork, prescriptions, medication reviews, results, medical reports (as appropriate), and visiting patients at their home as and when appropriate. You are expected to participate in practice activities including meetings and audits, as well as taking on a role in the development of the practice as outlined in your contract. Primary key responsibilities The following are the core responsibilities of the Salaried GP in delivering health services. a. The delivery of highly effective medical care to the entitled population b. The provision of services commensurate with the primary care contract c. To clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly referring patients to other services/agencies in a timely manner, utilising Practice and any other local guidelines d. Generic prescribing adhering to local and national guidance e. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care f. Effective management of long-term conditions g. To support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care h. Processing of administration in a timely manner, including referrals, repeat prescription requests and other associated administrative tasks i. Review medications following the appropriate policies, NICE/local clinical guidelines and local care pathways j. Maintain accurate clinical records in conjunction with good practice, policy and guidance k.
