This role will initially be for a two-year fixed term contract, providing four clinical sessions per week, two sessions on a Wednesday and two sessions on a Friday.The role includes proportionate participation in an on-call rota with the other GPs, approximately one on-call session every other week. Informal visits to the practice can be arranged with the practice manager by appointment. Please email alexandra.jones8@nhs.net. ### Main duties of the job Working as part of our multidisciplinary team, you will support and care for a diverse patient group.The role will include all general duties and responsibilities associated with a GP working within primary care, as well as: - Undertaking a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion; - Making professional, autonomous decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the organisation; - Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems; - Screening patients for disease risk factors and early signs of illness; - Developing care plans for health in consultation with patients and in line with current practice disease management protocols; - Providing health education; - Recording clear consultation notes to a high standard; - Collecting data for audit purposes and complying with QOF requirements; - Delivering enhanced services; - Compiling and issuing computer-generated acute and repeat prescriptions; - Participating in professional meetings throughout the year as required; - All general duties and responsibilities associated with a GP working within primary care. ### About us We are a two-site training practice, based in the semi-rural villages of Hatfield Peverel and Boreham, serving a patient population of 11,400. We are a SystmOne Practice with a consistently high QOF achievement and a CQC rating of Good. We are part of the Aegros PCN with Lt. Waltham & Great Notley, Douglas Grove Surgery and the Beacon Health Group. Our team is made up of four GP partners, four salaried GPs, a paramedic practitioner, a nurse prescriber, three practice nurses, two HCAs, a phlebotomist. We are further supported by PCN staff including child and adult mental health practitioners, a pharmacist, a physiotherapist and a social prescriber. Additionally we have a well established non-clinical team of managers, administrators, dispensers and receptionists. Staff wellbeing is very important to us.
