We would ideally like a GP alongside the Lead GP and Lead ANP for the Enhanced Care Team, deputising for the Lead GP when required. The role will predominantly deliver acute visits to our housebound patients but will also be involved in the training and support of our aligned Care Home staff, as well a broader education and development for the communities served by the PCN. ### Main duties of the job Provide 3 to 4 acute visiting sessions- Monday, Wednesday, Thursday, Optional Friday 09.00-13.00 A minimum of 5 visits per session would be expected, subject to clinical complexity and geographical distance. Provide cover when required for the Care Home round calls. Working with the Lead GP for Enhanced Care Team. Provide palliative care and lead the PCN palliative MDT. Provide remote consultation by phone where appropriate. Provide clinical advice and guidance to the multi-disciplinary Enhanced Care Team. Training and support of our aligned Care Home staff Supports the development of PCN service delivery within the Enhanced Care Team.
