### Job summary

Our Enhanced Care Team is a multi-disciplinary team based at Treatment Centre, Shelley Manor Medical Centre that provided support to our aligned Care Homes, our proactive frailty support and acute visiting service to housebound patients.

We would ideally like a GP alongside the Lead GP and Lead ANP for the Enhanced Care Team, deputising for the Lead GP when required. The role will predominantly deliver acute visits to our housebound patients but will also be involved in the training and support of our aligned Care Home staff, as well a broader education and development for the communities served by the PCN.

### Main duties of the job

Provide 3 to 4 acute visiting sessions- Monday, Wednesday, Thursday, Optional Friday 09.00-13.00

A minimum of 5 visits per session would be expected, subject to clinical complexity and geographical distance.

Provide cover when required for the Care Home round calls.

Working with the Lead GP for Enhanced Care Team.

Provide palliative care and lead the PCN palliative MDT.

Provide remote consultation by phone where appropriate.

Provide clinical advice and guidance to the multi-disciplinary Enhanced Care Team.

Training and support of our aligned Care Home staff

Supports the development of PCN service delivery within the Enhanced Care Team.

Maintain the highest clinical standards at all times and practices in accordance with accepted good medical practice and service standards.

BEC Treatment Centre working out of the Treatment Centre at Shelley Manor Medical Centre supported by GPs, ANPs, Respiratory Nurse and HCAs.

First Contact Physiotherapy Practitioner service.

Digital Lead, Digital Care Coordinator, Digital Champions.

Pharmacy team comprising of Clinical Pharmacists, Pharmacy Technicians and Care Co-ordinator.

Mental Health practitioners.

Health & Wellbeing Coaches and Social Prescribers within Help and Care team.

What we need from you -

Essential

GMC registered.

MBBS or equivalent medical degree

MRCGP

Experience of S1 and understanding of working access multiple modules.

Evidence of interest in relevant additional clinical skills.

Enhanced DBS Clearance.

Owner of vehicle and a hold a full UK driving licence

Excellent interpersonal and communication skills, using a range of methods, e.g. verbal, written, record keeping.
