### Job overview

We are recruiting to both our Buckingham and Aylesbury bases.   You will be working with  a Multidisciplinary team of Physiotherapists, Occupational Therapists, Intermediate Care Nurses, Community Healthcare Assistant Practitioners and Healthcare Assistants in a clinical and management role providing specialist assessment and treatment, supporting patients at home primarily working on admission prevention but also supporting  timely  hospital discharge. Our Urgent Community Response service and Frailty Virtual ward team are key partners and we work closely together to ensure care is delivered as close to home as possible.

Occupational therapy plays a key role in the rehabilitation and maximising independence of the  wide variety of patients we support on our service.

We work closely with Community nursing teams, Single Point of Access,  Primary care and Adult Social Care and there are lots of opportunities for joint working.

As Integrated Neighbourhood teams develop we are committed to RRIC playing an integral part in supporting our local population.

### Main duties of the job

Your main duties include:

- Providing advanced skilled, professional triage and expert assessment of health needs and relevant therapy case management, especially for those with complex needs and at high risk of unplanned admission to hospital.
- Educating and supporting members of the multi-disciplinary team to intensively case manage these patients.
- Working with a range of professionals across the system in order to ensure care is appropriate and delivered in a timely manner with the aim of preventing hospital admission, supporting early discharge and reducing GP contact.
- Demonstrating your professional skills and competence in leadership, care planning / co-ordination, and care and treatment delivery.
- Providing professional and operational leadership for the local team
- Engaging with RRIC Clinical Leads to offer and find ways for continuous service development and deputise for the Clinical Lead as needed
- Supporting all staff to develop their knowledge and skills.
Staff Management skills and experience of leading a team are essential 
    A full driving licence and access to a car as all visits are in the community

### Experience

**Essential**

- Relevant post registration NHS / Adult Social Care specialist/clinical experience and competence
- In-depth experience of inter-agency and partnership working
- Experience of working as advanced practitioner/case manager
- Leading complex care co-ordination
- Management of complex Long Term Conditions and care at end of life
- Knowledge of managing cognitive impairment and mental well being.
- Supporting self-care, self-management and enabling independence.
- Developing highly specialised programmes of care, providing advice concerning treatment of patients
- Professional practice and leadership including day-to-day staff management
- Experience of working in a community setting
- Experience participating in research or audit project.

- Valid and current UK drivers licence and access to a car with Business insurance for travel throughout Buckinghamshire

### Education Qualifications and Training

**Essential**

- Educated to Degree/ Diploma in relevant profession: Physiotherapy /Occupational Therapy / Paramedic Science / Nursing
- Registered Allied Healthcare Professional with HCPC - Physiotherapist /Occupational Therapist / Paramedic Or Registered Nurse with RCN
- Relevant post graduate clinical training e.g. Advanced History Taking & Assessment
- Recognised teaching or mentorship qualification equivalent to level 3
- Recognised management or leadership qualification
