An opportunity is available for a Band 5 Physiotherapist to join EPUT to work across Community services and Acute Wards at PAHT in a Band 5 Rotational role.

We currently have two full-time positions available.

### Main duties of the job

ROLE SUMMARY

- To take responsibility for providing, monitoring & developing the Physiotherapy Service within the west Essex Community: Helping patients to engage as independently as possible in activities which enhance their health & wellbeing.
- This role supports a wide range of presentations supporting both rehabilitation and long term care pathways in the community. This includes inpatient and acute setting. Includes seeing patients in their own homes, 24 hour care facilities clinics and community and acute wards.
- To manage a caseload of patients with health & social care needs, using evidence based, client-centered principles to assess, plan, implement & evaluate interventions
- Opportunity to rotate through our community, early supported stroke services and acute partner at PAH.
- The community team of West Essex is part of the wider Hertfordshire and West Essex Integrated Care System supporting Out of Hospital Care to support West Essex Residents. The post holder will be working as part of the Primary Care Aligned Team (PACT) supporting Harlow Primary Care Networks with a focus on prevention of admissions, supporting safe discharge as well as provision for rehabilitation for the West Essex population.
- Flexible working is required, as at times you will be expected to work across sites in our other West Essex localities depending on service needs.

### Detailed job description and main responsibilities

KEY RESPONSIBILITIES CLINICAL

- To be professionally and legally accountable for all work undertaken. To work within the standards and guidelines of the team, the Trust, CSP, and HPC code of ethics.
- To undertake physiotherapy assessment of clients with diverse needs through subjective questioning, objective assessment and other written material available.
- To prioritise factors involved in the clients presenting condition through clinical reasoning based on knowledge of current professional opinion, client’s prognosis and other physical, social emotional and mental health issues.
- To set goals for treatment with clients and/or their carer and ensure that care is client centred and agreed by the client and/or their carer.
- To set and maintain a high standard of clinical practice which is evidence based where possible, using standardised assessment and outcome measures, where appropriate
- To manage a caseload of clients with simple to complex needs with supervision and guidance as necessary from senior staff. To successfully monitor, evaluate and review physiotherapy treatment/interventions within a multi-disciplinary framework and to modify as necessary.
- To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care.
- To have the ability, knowledge and skills required for moving and handling people in community environments.
- To ensure client and service records are recorded and maintained on IT databases and that client documentation is according to trust and local policies.
- To prescribe, delegate and monitor the work of therapy assistants
- To educate and teach the relevant carers/health workers to participate in on-going rehabilitation care programmes for clients, as appropriate.
- To actively be involved in the implementation of new specialist clinical services within the Team, with support and guidance from senior physiotherapy staff.
- To manage clinical risk in own caseload and ensure that rehabilitation assistants and students comply e.g. manual handling risk assessment and local policy regarding lone working.
- To maintain and update a Continuing Professional Development Portfolio (CPD).
- To evaluate your practice through audit and outcome measures with support of senior physiotherapists when required.
- To work at other sites in the trust if required

ADDITIONAL DUTIES

In addition to the above duties you will also be expected to perform the below key activities in line with your job role;

- Complete mandatory training in line with Trust policy and procedures

- To participate in the staff appraisal process and to undertake for any staff you manage

- To keep yourself updated on all matters relating to Trust policy

- To provide management supervision where appropriate
