### Job overview

To contribute to the provision of Respiratory Services in Luton to meet the needs of people with respiratory conditions, including COPD, Asthma, ILD and Bronchiectasis who require community services.

Able to demonstrate an understanding of the needs of the patient group and will have experience supporting this client group in either a hospital or community setting. You will be able to deliver high quality care and demonstrate clinical decision making, working independently and as part of the wider multidisciplinary team. The Community Respiratory Service is committed to the continuing professional development of its staff to support patient outcomes and a high level of patient satisfaction.

### Main duties of the job

To support the vision, purpose and objectives of the Community Respiratory Services.

To be accountable for the management of patients in your care, to ensure a high standard of evidence based clinical care is maintained

Ensure all electronic based patient records are kept up to date in line with the Trust’s record keeping policy.

Work within EEC policy and service specific Standard Operating Procedures.

To work in partnership with patients encouraging appropriate use of the resource, including social care agencies and the voluntary sector, raising the profile of respiratory needs across Luton, considering the diversity of population and the promotion of respiratory health.

Work with colleagues in the Acute Trust and Primary Care to identify patients who need the support of the multi-disciplinary team to prevent frequent hospital admissions.

To work within current clinical guidelines and evidence based practice to develop a gold standard service.

### Detailed job description and main responsibilities

The post holder will maintain verbal and written communication with patients’ relatives and carers and be able to maintain effective communication with external agencies including acute sector, primary care and the voluntary services.

Communicating sensitive condition related information to patients and relatives requiring empathy and reassurance.

The post holder will be expected to participate in relevant meetings and undertake teaching sessions to staff and patient groups

Ensure compliance with agreed governance systems across the service

Be responsible for maintaining own mandatory training compliance, annual PDP and continuing professional development to encourage professional curiosity.

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

The post holder must adhere to the Trust risk assessment and risk management processes

To ensure own performance standards are maintained and the direct reports meet performance requirements including clinical competencies.

Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

The post holder must adhere to infection control policies and procedures.

It is a condition of your employment that you are currently registered with the NMC and it is your responsibility to maintain your professional registration.

Undertake mandatory training and any other training relevant to the role as required by East of England Community Health and Care NHS Trust.

The post holder must participate in clinical and other audits as required.

Provide patients and their families /carers with information on standards they should expect from the service and ensure the service has mechanisms to capture user and carer experience

Contribute to and support the implementation of agreed pathways and protocols for respiratory care.

You must have appropriate UK professional registration.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

## Person Specification

### Experience

**Essential**

- Experience of partnership and multi- disciplinary working
- Competent in caseload management
- Evidence of post graduate experience caring for patients with a range of Respiratory conditions

**Desirable**

- Experience of working with patients in a Pulmonary Rehabilitation setting

### Knowldege and skills

**Essential**

- Excellent communications skills
- Standard keyboard skills and ability to communicate through IT using packages such as Word and Outlook
- Able to work unsupervised
- Ability to work flexibly as part of a team
- Understanding of when to seek advice

**Desirable**

- Audit experience

### Qualifications and Training

**Essential**

- Registered Nurse
- Relevant post registration Respiratory course
- Recognised qualification at degree level in nursing
