This support and education will help enable and empower this group of patients to take responsibility for their kidney disease and its management. The post holder will work closely with the renal clinical teams at St George’s University Hospitals NHS Foundation Trust and Young Adults/Transition Team to design and implement changes in the care of young adults living with kidney disease to ensure the best possible care for this group.
