Working autonomously within the scope of practice and under the guidance of registered clinicians when required, the post holder will assess, implement, monitor, and evaluate personalised rehabilitation and falls prevention plans, including for patients with complex physical, psychological and social needs. They will use effective communication, motivation, and problem-solving skills to achieve positive patient outcomes. Key responsibilities include delivering delegated therapy and nursing interventions, progressing treatment plans, reporting changes in patient condition, and seeking clinical advice when appropriate. The role also involves delivering education to patients, carers, and staff, facilitating individual and group rehabilitation sessions, promoting safe moving and handling practices, and providing guidance on adaptive equipment to maximise patient independence. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.
