### Job summary

Working with a supportive community team, you'll deliver high-quality nursing care so that patients receive safe, compassionate support when they need it most. You'll join an Intermediate Care Team focused on helping people recover, avoid hospital admissions, and maintain independence. This role offers a mix of hands-on care and leadership, with opportunities to guide junior staff and develop your clinical skills in a varied setting. Your work will make a direct difference to patients and their families during important moments in their care journey.

### Main duties of the job

What you'll do:

- Assess, plan, deliver and review care for a group of patients
- Provide safe, person-centred nursing care in community bedded units
- Support and supervise junior staff and learners during shifts
- Communicate clearly and sensitively with patients, relatives and colleagues
- Take responsibility for running the service in the absence of senior staff

What we're looking for:

- A registered nurse qualification and current nursing experience
- Confidence working both independently and as part of a team
- Strong communication and leadership skills
- A caring, calm approach in complex or sensitive situations
- Willingness to learn, develop and support others

### Qualifications

**Essential**

- RGN/RN/Diploma Studies
- Be willing to undertake further training and development

**Desirable**

- Completion of post qualification preceptorship

### Knowledge & Experience

**Essential**

- To have recently worked within a nursing capacity
- Demonstrate an understanding of the MDT

**Desirable**

- Community Hospital experience
- Demonstrate an interest in Intermediate Care
- Demonstrate an understanding of evidence based practice
- Have an interest in audit and maintaining standards
