This role offers the opportunity to deliver high-quality, patient-centred nursing care within patients’ own homes, supporting independence and preventing avoidable hospital admissions. You will work as part of a multidisciplinary neighbourhood team, managing a varied and rewarding caseload across the community setting Our service operates 08:00–18:00, 7 days a week. You will be responsible for assessing, planning, implementing and evaluating evidence-based nursing care, including: - Complex wound care and pressure ulcer management - Leg ulcer care and Doppler assessment - Catheterisation (male/female/suprapubic) - IV, subcutaneous and intramuscular medication administration - PICC/Hickman line management - Palliative and end-of-life care including syringe drivers and symptom control - Continence and bowel management - Supporting referrals and coordinating multi-agency care You will work autonomously while being supported by senior colleagues, and will contribute to clinical governance, audit and service improvement We are committed to delivering compassionate, high-quality community care,  supporting professional development and career progression, collaborative integrated working and making time matter for patients and staff Please note this role is not suitable for Newly Qualified Nurses Applicants must have full UK driving licence and vehicle access ### Main duties of the job To take responsibility for assessing, planning, implementing and evaluating evidence based care for patients.
