Full Time RMN / Care Coordinator

We are looking to recruit an experienced mental health nurse to our small busy team.

### Main duties of the job

The role includes:

Medication Management

Caseload Management and Care Coordination

Individual and group work to support assessment and treatment

Supervising support staff and students

### Detailed job description and main responsibilities

Medication management including handling controlled drugs

Managing a complex caseload

Individual and group interventions to support assessment and treatment

Collaborative care planning using Dialog +

Completing cognitive assessments

Physical health monitoring

### Applicant requirements

You must have appropriate UK professional registration.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

- - Ability to work independently and autonomously.
- Effective Medication Management skills
- Developing care plans with individuals and carers
- - Ability to undertake comprehensive risk assessment and implement risk management plans.
- - Ability to network, collaborate and liaise with other agencies.
- - Effective Physical health monitoring skills and knowledge of when to escalate concerns

**Desirable**

- Experience of using Dialog + and co-producing care plans

### Experience

**Essential**

- - Post registration experience as a Mental Health nurse preferably in a mental health setting working with older adults with functional and organic mental health needs.

**Desirable**

- Experience of working with Older Adults
- Experience of carrying out assessments independently including risk assessment and mental capacity assessment
- Experience of Care Coordination
- Experience of carrying out individual work and group work

### Qualifications

**Essential**

- - Dip/BSc (Hons) in Mental Health Nursing
- - Currently Registered with appropriate professional body

**Desirable**

- Phlebotomy
- Evidence of further relevant post qualification training
- Practice Assessor training
