The post holder will work as part of a team of nurses dedicated to Home Parenteral Nutrition (HPN) patients.  They will require clinical skills and expertise in the assessment, preparation and training of patients and their families for Home Parenteral Nutrition, Intra-vascular devices and Intestinal Failure management at home. They will undertake the follow up of patients in the home and appropriate outpatient’s clinics.  They will give clinical advice to patients about their Home Parental Nutrition related problems.

The post holder will be able to work unsupervised with the support of a Team Leader.

- Promote a healthy and appropriate lifestyle.
- Be a link between the hospital and the community.
- Liaise with the multi disciplinary team in the hospital and appropriate Primary Care Trust’s.
- Supervise patients and their relatives in their home environment.
- Undertake Home Parental Nutrition administration and associated therapies in the patient’s home as required.
- Support patients and relatives through their care.
- Attend pre assessment clinics prior to the patients discharge on Home Parental Nutrition.
- Undertake clinical procedures and tests in the patient’s home.
- Handling bodily fluids when necessary.
- Working in different conditions on occasions, adhering to the “lone worker policy”.

Inform Home Parental Nutrition patients and their relatives as to treatment options available.
Teach staff, both hospital and community based, about the Home Parental Nutrition role.
Teach staff in other hospitals to undertake various procedures.
Be a link nurse to the multi disciplinary community team if necessary.
Train patients to undertake connection and disconnection of Parental Nutrition and other related procedures.
Facilitate the support of patients and relatives.
Assist in any new expansion within the directorate.
Keep updated in all areas of I.T.
Develop both personally and professionally.
