### Job overview

The Quality and Safety Facilitator Midwife is a role that is integral to promoting and supporting a positive culture of learning. It will provide essential support to the Maternity and Neonatal team through ensuring the co-ordination of clinical risk across the service on a daily basis. The individual will work with all grades of clinical staff and managers across the multi-disciplinary team (MDT) to identify risks, report risks appropriately, be involved in the incident reporting and management system, ensuring that necessary actions follow, and learning takes place. The individual will contribute to the healthcare governance agenda together with the Quality and Safety Team.

### Main duties of the job

To support the provision of healthcare to women and new-borns treated by Salisbury NHS Foundation Trust by supporting the delivery of mechanisms in place, in respect of risk management and patient safety incidents.

To receive, review, grade and investigate all reported clinical incidents and near misses together with the divisional and central risk management team.

To ensure that any immediate actions from incidents are escalated as appropriate.

To ensure effective communication (written and verbal) with all staff within maternity services providing feedback to staff through a variety of methods and ensuring all staff involved in the incidents are signposted to appropriate support services.

To maintain an incident reporting system which fulfils internal, and external reporting requirements. Ensuring that incidents are investigated at an appropriate level and resulting improvements can be evidenced

To collate and maintain a database of incident data and use incident trend analysis to identify trends from Datix incidents. Analyse and triangulate information; identify and implement mitigation and solutions to reduce risk.

To promote a ‘Just Culture’ approach within patient safety incidents.

To lead by personal example, role-modelling when working with others and sharing expertise in clinical practice.

- NMC Registered Midwife
- 3 years recent NHS clinical experience in acute and community midwifery settings
- Datix (incident reporting) skills
- Awareness of clinical governance and NHS Safety Framework
- Evidence of facilitating learning

- IT skills

**Desirable**

- Evidence of continued professional development
