We have an exciting and rare opportunity for you to join our team as a Quality and Clinical Excellence Facilitator

This is a new post that will support the Quality and Clinical Excellence Team to drive and sustain improvements across the Trust.

You will work alongside our Clinical Leads for Quality to support a wide range of portfolios, including Deteriorating Patient, Nutrition & Hydration, Documentation, Falls, Deconditioning and Dementia.

We will provide training and education to fully equip you with all aspects of the job role.

Due to the nature of the role we are looking for a candidate with substantial NHS experience.

The role will require cross site working and so it is essential that you can travel across all Shropshire Community bases

### Main duties of the job

The post holder will play a key role as a member of the Quality and Clinical Excellence Team, working across all clinical areas to improve the quality of care in all key nursing and patient safety domains. The postholder will be pivotal in progressing work around CQUIN achievement and ensuring the quality of care is sustained and /or improved as a result.

The post holder must have proven experience in implementing quality improvement initiatives and of engaging with the multi-disciplinary team to develop and implement mutually driven schemes. This includes learning from patient experience, adverse incidents and complaints.

A key remit of the role is to help drive and sustain quality improvements. This post holder will facilitate and co-ordinate a portfolio of quality improvement work across the Trust supporting the Trust’s delivery improvements, this includes but is not limited to improvement initiatives linked to falls and deconditioning, deterioration, nutrition and hydration, medicines safety, risk assessments and documentation.

The post holder is also expected to contribute to the ward/clinical area accreditation programme and provide advice and support to teams about quality & safety.

- Ability to meet the travel requirements of the post (The postholder will be required to travel to any location as required by the organisation).

### Experience

**Essential**

- Comprehensive knowledge of CQUINs and health information systems
- Evidence of previous work in Quality Improvement in Healthcare environment
- Knowledge of performance and project management frameworks and assurance cycles.
- Experience of planning for, and evaluating change in a healthcare setting.
- Experience of project principles, techniques and tools.

### Knowledge & Skills

**Essential**

- Knowledge of presenting complex information using Microsoft Excel, Microsoft Word and Microsoft PowerPoint, including the formation of SPC charts, tables and other visual representation of performance
- Experience of involvement/leading service improvement
- Attention to detail combined with the ability to extract key messages from complex analysis
- Problem solving skills and ability to respond to sudden unexpected demands
- Highly developed negotiating and influencing skills, overcoming barriers to understanding and acceptance, and reaching agreements
- Evidence of driving service improvement for healthcare services

### Training & Qualifications

**Essential**

- Relevant qualification to degree level or equivalent experience and demonstrable specialist knowledge and expertise (to post-graduate diploma level or equivalent) in: Project, Programme, Change Management Practitioner Qualification (Prince2 Project Management Practitioner, or Managing Successful Programmes Practitioner or Change Management Practitioner, or equivalent)
- Evidence of ongoing CPD
