The post holder will support the delivery of the Prostate Cancer Supported Self‑Management (SSM) pathway, working closely with the Prostate CNS team to ensure safe, effective, and patient‑centered care. They will act as a key point of contact for patients on remote monitoring pathways, coordinate PSA tracking and safety‑netting processes, and provide high‑quality information, support, and signposting. The role involves the completion of holistic assessments, Implementing personalised care plans and signposting patients, carers and their families to support agencies highlighted through the assessment process, supported by the CNS team.
