### Job summary

The PCN Transformation & Development Manager will lead, coordinate, and drive delivery of the PCN DES and wider transformation programmes across the Federations three Primary Care Networks. The role ensures that PCN objectives and Federation strategic aims are aligned, with member practices at the heart of all planning, decision making, and service development.

The postholder will work proactively and collaboratively with practices, Clinical Directors, the Integrated Neighbourhood Team (INT), and wider system partners to shape and deliver neighbourhood level priorities. This includes incorporating SNP and MNP neighbourhood development work, ensuring PCN activity is fully aligned with emerging neighbourhood models, INT maturity, and the expectations set out in the 10 Year Health Plan.

The role requires strong partnership working with the GP Federation, ensuring consistent communication, coordinated planning, and a unified approach to neighbourhood level development, population health management, and service improvement. The postholder will operate with a high degree of autonomy, exercising sound judgement, initiative, and leadership across multiple organisations while fostering a culture of shared ownership and collaborative working.

Experience in primary care, general practice operations, and working with ICB partners is essential to ensure effective navigation of system structures, governance, and transformation programmes.

### Main duties of the job

1. Strategic Leadership & PCN Business Planning

- Leading development, delivery and annual refresh of a PCN strategy and business plan.
- Translate DES requirements into clear operational plans with measurable outcomes.
- Centralisation of DES-related workstreams wherever appropriate, reducing duplication and variation.

2. Programme & Transformation Management

- Lead planning, implementation and evaluation of transformation programmes including personalised care, digital and data initiatives, workforce development and neighbourhood level projects.
- Develop programme documentation, risk logs, delivery plans and progress reports.

3. Workforce Leadership & Line Management

- Provide day-to-day leadership and supervision, for ARRS staff, ensuring clarity of role, workload management and high quality performance.
- Support ARRS staff to work consistently across practices, embedding standardised processes and shared operating models.

Key Responsibilities

- Work with the Federation HR Manager to ensure robust workforce planning, recruitment, supervision, and development.

- Build strong, trusting relationships with member practices, ensuring transparent communication and shared ownership of PCN priorities.
- Actively engage practices in shaping PCN strategy, DES delivery, and transformation programmes.

## Person Specification

### Experience

**Essential**

- An appreciation of the nature of GPs and general practice.
- Proven experience in primary care, PCN Management, or wider NHS transformation roles.
- Strong Leadership skills with the ability to support, influence, and motivate Clinical Directors and multidisciplinary teams.
- Excellent programme management skills with a track record of delivering complex projects.
- Ability to bring strong relationships with practices and system partners.
- An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners.

**Desirable**

- Experience line managing staff, ideally within a primary care or community setting.
- Experience working within a GP Federation or multi-practice environment.
- Experience of writing and contributing to high quality reports on projects undertaken for internal and external stakeholders.
