The tertiary service will initially operate clinically across a variety of locations including community, hospital sites and virtually and will involve support of schools, teaching and training, co-design of pathways, tools and workshops and active support of communities of practice.

The clinical model is based on a service specification from national workshops in Wales and integrated care models recommended nationally.  It also aligns with the C & V Rehabilitation Model.

### Main duties of the job
