### Job summary

We are seeking a motivated and caring Practice Nurse to join Withington Medical Practice on a part-time basis (2 days per week). We are flexible regarding working days and will do our best to accommodate the successful candidates availability where possible.

The post holder will be part of a wider multidisciplinary team, working alongside GPs, physician assistants, pharmacists, and administrative colleagues. The role suits a registered nurse who is confident in primary care work, committed to patient-centred practice, and interested in long-term condition management and health promotion.

### Main duties of the job

The Practice Nurse will be expected to provide a range of clinical services, including:

Ongoing care and monitoring of patients with long-term conditions such as diabetes, asthma, COPD, hypertension, and cardiovascular disease

Cervical screening services

Delivery of childhood and adult immunisations

Health promotion and lifestyle support

NHS Health Checks

Venepuncture and other clinical procedures as required

Infection prevention and control duties

Accurate and timely clinical documentation

Working as part of the multidisciplinary team to support safe and effective patient care

Contributing to service development and quality improvement within the practice

### Job responsibilities

To provide and maintain a high standard of nursing care for patients attending the practice, working in line with professional standards, clinical guidelines, and practice policies to ensure safe, effective, and patient-centred care.

Key responsibilities

Clinical responsibilities Deliver evidence-based nursing care to patients across the practice population. Carry out chronic disease management reviews in accordance with national and local guidance. Administer immunisations and vaccinations in line with current protocols and best practice. Undertake cervical screening where appropriately trained and competent.Support health promotion and disease prevention activities. Assist patients in the management of long-term conditions, encouraging self-care and improved health outcomes. Participate in routine screening programmes and NHS Health Checks. ## Person Specification

### Experience

**Essential**

- Registered Nurse with current registration with the Nursing and Midwifery Council (NMC) and a valid PIN.
- Experience of delivering safe, effective patient care in a clinical setting.
- Experience of administering vaccinations and immunisations.
- Experience of undertaking clinical assessments and contributing to patient care plans.
- Experience of undertaking cervical screening (smear tests)
- Experience of maintaining accurate and timely clinical records.
- Strong communication and interpersonal skills with patients and colleagues.
- Ability to work both independently and as part of a multidisciplinary team.
- Good organisational skills and ability to manage competing priorities.
- Understanding of infection prevention and control principles.
- Awareness of safeguarding responsibilities and procedures.
- Competent IT skills and ability to use clinical systems confidently.
- Willingness to develop skills and adapt within a busy primary care environment.

**Desirable**

- Previous experience working in General Practice or Primary Care.
- Experience managing patients with long-term conditions.
- Knowledge of QOF (Quality and Outcomes Framework).
- Experience using EMIS Web or similar clinical systems.
- Understanding of current NHS priorities in primary care.

### Qualifications

- Registered Nurse qualification with current NMC registration and valid PIN.
- Evidence of continuing professional development relevant to nursing practice.
- Experience in vaccination and immunisation delivery.
- Experience in clinical assessment and patient-centred care delivery.
- Experience in maintaining clear and accurate clinical documentation.
- Ability to work effectively in a busy healthcare environment with a compassionate and patient-focused approach.
- Experience in cervical cytology

**Desirable**

- Training in chronic disease management or long-term condition clinics.
