Applicants must have experience of working in a GP Practice. The successful applicant would be working alongside our current nursing team, and be responsible for the delivery of Diabetes practice nursing services to our patient population. ### Main duties of the job Your main responsibilities will be the routine evidence based care of our diabetic population, including annual reviews incorporating the local prescribing guidelines as well as following up patients with more complex diabetic needs. The ideal candidate would have current Primary Care experience and would be a non-medical prescriber but this is not essential. ### About us Pontesbury & Worthen Medical Practice is a developing and growing practice with two sites, situated in the rural villages of Pontesbury and Worthen, just south of Shrewsbury providing medical care for approximately 10500 patients.
