You must have a flexible and adaptable approach to a varied and high volume workload which includes triage and face to face appointments. The ability to multi-task and to work under pressure is essential. The successful applicant would be working alongside our current multidisciplinary team, and be responsible for the delivery of nursing services to our patient population. The role will include managing areas such as acute care, health promotion, chronic disease management and health prevention. ### Main duties of the job In consultation with patients and in line with current practice disease management protocols, develop care plans for health management, offering support and health education. Responsible for a number of clinical areas such immunisation clinics and well men and women health checks. Making appropriate and timely referral of patients for further opinion and treatment. You will be the first point of contact for patients presenting with undifferentiated, undiagnosed problems, using skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis management plan via triage and face to face appointments. You will report to the Partners. Training will be provided. ### About us Montgomery Medical Practice is a rural, dispensing GP Practice with around 8100 patients situated on the border of Montgomeryshire and Shropshire. Despite its idyllic rural location Montgomery is within a short 30 minute drive of Shrewsbury and an hour and a half from Birmingham. We have a multidisciplinary team that includes Physician Associates, Nurse Prescriber, Practice Nurses and Health Care Assistants. We are a training practice.
