## Job Content

### Job overview

The Population Health Management (PHM) team is a system-supported programme working across Lewisham in collaboration with local health and care partners. The programme uses data and analytics to understand the health and care needs of the population, identify inequalities and unwarranted variation, and support improvements in health outcomes and service delivery.

The team is building on a newly developed Snowflake data environment, currently in the early stages of release, bringing together integrated health and care datasets into a unified analytical environment. This will support a more comprehensive understanding of population health and enable the development of new analytical tools, metrics and approaches.

The Population Health Data Analyst will contribute to the development and use of this environment, including scripting, querying, developing metrics and supporting the integration and analysis of health and care data.

- Strong analytical and programming skills, particularly SQL and Snowflake, with experience of Power BI.
- Experience of developing metrics, queries and analytical products.
- Experience of working with integrated health and care datasets.
- Experience of supporting population health, neighbourhood health, proactive case finding or service improvement would be advantageous.
