We are seeking an enthusiastic and motivated Registered Nurse (Band 7) to join our forward‑thinking Pre‑assessment and Population Health Management Team at West Middlesex Hospital.

This is a unique and exciting opportunity to establish and develop the Trust’s first Population Health Management Clinic, while also playing a key leadership role within the Pre‑assessment (POA) service. The post holder will work collaboratively with hospital and community teams to deliver a high‑quality, patient‑centred service aimed at improving both individual surgical readiness and wider population health outcomes.

The role combines clinical expertise, leadership, and service development, with responsibility for assessing patients prior to surgery, identifying and managing risks, and ensuring patients are optimised for safe procedures. This includes reviewing medical history, performing clinical assessments, arranging appropriate investigations, and providing education to support informed decision‑making.

In parallel, the post holder will lead on prevention, early intervention, and lifestyle modification, supporting patients to reduce risk factors such as smoking, poor diet, and inactivity, thereby improving long‑term health outcomes and reducing health inequalities.

- Assess the health status of adult elective surgical patients, obtaining a comprehensive medical, psychological, and social history, involving carers/relatives where appropriate
- Perform clinical assessments to determine fitness for anaesthesia and surgery, making autonomous decisions regarding suitability to proceed
- Undertake clinical interventions such as phlebotomy and ECG recording in accordance with Trust policies
- Request, perform, and interpret investigations in line with agreed protocols and multidisciplinary guidance
- Demonstrate advanced analytical and clinical judgement to develop appropriate care plans
- Identify and manage perioperative risks, including co-morbidities and medication optimisation
- Provide clear, accurate information to patients regarding procedures, risks, benefits, and recovery, supporting informed consent
- Discuss discharge planning and set expectations, particularly for day-case surgery pathways
- Contribute to the development and implementation of pre-assessment protocols, pathways, and guidelines
- Review and evaluate results of investigation/care promptly to ensure patient’s condition and safety is optimised preoperatively to avoid inappropriate admissions and delays in surgery being performed.
