This is a unique and exciting opportunity to establish and develop the Trust’s first Population Health Management Clinic, while also playing a key leadership role within the Pre‑assessment (POA) service. The post holder will work collaboratively with hospital and community teams to deliver a high‑quality, patient‑centred service aimed at improving both individual surgical readiness and wider population health outcomes. The role combines clinical expertise, leadership, and service development, with responsibility for assessing patients prior to surgery, identifying and managing risks, and ensuring patients are optimised for safe procedures. This includes reviewing medical history, performing clinical assessments, arranging appropriate investigations, and providing education to support informed decision‑making.
