We are seeking a highly motivated and enthusiastic Band 5 Float Physiotherapist to join our therapy rehabilitation teams within the North locality. Vacancy to be based at Chester-Le-Street hospital though this is a float role, involving cross-site cover into Weardale Community Hospital and Shotley Bridge Hospital. We also have x2 community hospitals within the South locality - Sedgefield Community Hospital and Richardson Community hospital. There is also the expectation that staff would cross-site cover into these hospitals if staffing requires, though this can be discussed further during the interview. The post holder will independently manage a caseload and provide assessments to adults referred to the team who present with a range of physical dysfunction/or disabilities resulting from injury, illness, surgery or chronic disease, within a range of community settings and establishments (including Rehabilitation Wards/Units, Clients Homes, Care Homes) On an on-going basis the post holder will assess, diagnose, implement and evaluate individual programmes of care for patients. The post holder will work within a multi-professional team (Consultants, GPs, nursing staff, Occupational Therapists, Social workers and other AHPs) regarding the management and discharge of patients under your care.
