### Job summary

We have an exciting opportunity to join our team. We are seeking a highly motivated and enthusiastic Band 5 Float Physiotherapist to join our therapy rehabilitation teams within the North locality.

Vacancy to be based at Chester-Le-Street hospital though this is a float role, involving cross-site cover into Weardale Community Hospital and Shotley Bridge Hospital.

We also have x2 community hospitals within the South locality - Sedgefield Community Hospital and Richardson Community hospital. There is also the expectation that staff would cross-site cover into these hospitals if staffing requires, though this can be discussed further during the interview.

Applicants must be prepared to participate in a 6 day week service, covering bank holidays and Saturdays on a rota basis.

### Main duties of the job

This post is for a Band 5 Float Physiotherapist full time.

The service will work over 6 days a week.

The successful applicant will independently manage a caseload providing holistic, short term, goal led intervention following a comprehensive assessment. These patients will present with a wide range of problems and will allow you to develop your knowledge

### Job responsibilities

To work as a member of the clinical team of physiotherapists, occupational therapists and therapy assistants within community therapy services.

The post holder will independently manage a caseload and provide assessments to adults referred to the team who present with a range of physical dysfunction/or disabilities resulting from injury, illness, surgery or chronic disease, within a range of community settings and establishments (including Rehabilitation Wards/Units, Clients Homes, Care Homes)

On an on-going basis the post holder will assess, diagnose, implement and evaluate individual programmes of care for patients.

The post holder will work within a multi-professional team (Consultants, GPs, nursing staff, Occupational Therapists, Social workers and other AHPs) regarding the management and discharge of patients under your care.

The post holder will work under the guidance of a senior therapist

## Person Specification

### Experience

**Essential**

- Relevant experience in physical rehabilitation including recent general rehabilitation
- Ability to work within a multidisciplinary team
- Evidence of communication skills within a wide variety of settings

**Desirable**

- Experience in working in a variety of settings.

### Qualifications

- BSc Physiotherapy or equivalent
- HCPC registration
- Continuing Professional Development Diary including evidence of reflective practice, training and clinical experience

### Special Requirements
