If you are an energetic and motivated Physiotherapist with a commitment to excellence in clinical practice and successful outcomes for people with a learning disability, then this role will appeal to you.

You will have the necessary clinical experience to manage complex cases within the community and work alongside primary care services. ### Main duties of the job

- To participate in the delivery of a countywide specialist LD Physiotherapy under the guidance of the lead physiotherapist by managing a caseload of clients with complex learning disabilities.
- To provide professional supervision to other staff /students as appropriate including appraisal and professional development particularly mandatory training.
- To contribute to the planning, development and evaluation of learning disability services, including contributing to defined projects as delegated by the LDP management team.
- As an autonomous practitioner to manage a specialist case load providing assessment, identification and treatment of the health needs of adults with a learning disability.
- To participate in multi-disciplinary/ multi-agency working within the LDP and with Key partners.

- The post holder will demonstrate specialist knowledge and skills and have undertaken appropriate post registration training.
- To have an understanding of Health and Social care policy and legislation for the service user group in relation to your professional practice.
- To independently prioritise, maintain and manage a complex specialist personal caseload within the context of the Learning Disability Partnership.
- To be proficient in the assessment, identification and treatment/intervention of the health needs of the learning disability population, including risk assessment, assessment of complex conditions e.g. mental health, epilepsy, challenging behaviour including forensic issues, palliative care etc.
- To understand and have experience in respiratory, musculoskeletal, and neurological areas of physiotherapy practice to enable good clinical decisions, including whether to treat or refer.
- To plan and implement specialist interventions that are enabling and client-centred.
- To apply a high level of understanding of the effect of disability when planning and implementing physiotherapy interventions.
- To set and communicate realistic and measurable clinical aims, using subjective, objective and functional markers to measure outcomes.
- To monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of the intervention.
- Lead in the co-ordination of complex case work as a case manager for individuals.
- To have a good understanding of the specific needs of the service user group including at times of crisis. e.g. during placement breakdowns, mental health and physical health relapses and periods of challenging behaviour.
- To work collaboratively with service users, their carers and families as appropriate, to make decisions relevant to the service users individual needs.
- To recognise the impact of a person’s communication needs and tailor interaction to meet this need.

### Experience

**Essential**

- Experience of working with adults with Learning Disability
- Experience of managing a caseload
- Experience of Physiotherapy assessment and intervention relevant to adults with LD

**Desirable**

- Post qualifying experience within a Community MDT
- Care Co-ordination in CPA/Case Management
- Experience in providing clinical supervision

### Skills/Abilities

**Essential**

- Effective written and verbal communication skills
- Effective organisation and time management skills
- Ability to prioritise your own work and the work of those you supervise

### Other Requirements

**Essential**

- Ability to Travel within a rural location
- To be skilled in selecting, setting up and adjusting specialist disability equipment

**Desirable**

- Able to manage the physical nature of community physiotherapy work

### Knowledge/Understanding

**Essential**

- MHA and MCA knowledge
- Understanding of clinical governance
- Awareness of current trends, legislation and guidance in support for people with learning disabilities and their carers
- Knowledge of the Enabling approach to support people to meet their full potential
- Evidence of continuous professional development

**Desirable**

- Ability to participate in research and Audit

### Education/Qualifications

**Essential**

- UK-recognised diploma or degree in physiotherapy
- Registered with Health and Care Professions Council

**Desirable**

- Experience of supervising staff
- Qualified Practice Placement Educator
- Additional clinical training relevant to specialism
