### Job overview

An opportunity has arisen for a motivated and enthusiastic Band 5 Physiotherapist within the Willow Therapy Unit.  The Willow Therapy Unit is a 48 bedded unit providing inpatient reablement for patients in Norfolk. This post will involve shift working across a 7 day service in a large integrated therapy team including Occupational Therapists, Physiotherapists and Assistant Practitioners.

We have one part time post available of 34.5 hours per week (3 shifts / 7am-7.30pm). Regrettably we are unable to offer sponsorship.

### Main duties of the job

- To contribute to the delivery of high standards of health care to patients within the Willow Unit.
- Be responsible for delivery of care in a reablement setting.
- To support an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.
- To support patients being as independent as possible within their functional limits.
- To work as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.
- To support and deliver innovation within discharge practice and pathways.
- To work with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from the unit.
- Be responsible for the delivery and co-ordination of care to a named group of patients.
- Work proactively within the team to achieve unit and trust goals and objectives.
- Work with and promote recognised policies, protocols, and guidelines.
- Assess, prescribe care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
- Work within the integrated team to facilitate early discharge from hospital.
- Work within the integrated team to prevent unnecessary admission to hospital.

### Experience

**Essential**

- Highly motivated & Able to provide high standards of care
- Ability to deliver effective patient care & inputting data/patient records
- Demonstrate that you can work in a team environment
- Work flexibly to accommodate patient/service needs
- Be able to kneel, bend & stoop, and work in cramped environments
- Be able to manoeuvre limbs of around 5-6 kg
- Be able to manoeuvre patients using handling aids

### Communication

- Able to communicate effectively in written and verbal English Language
- Demonstrates active listening skills
- Ability to remain calm under pressure
- Demonstrates tact and diplomacy
- Demonstrate ability to communicate with a wide range of people, including patients, relatives, health & social care professionals

- Demonstrates good telephone manner

### Qualifications

**Essential**

- BSc Hons in Physiotherapy  Or MSc in Physiotherapy, or equivalent
- Active HCPC registration

- Able to identify with the Trust’s commitment to safeguarding and promoting the welfare of children and young people/vulnerable adults
- Will be able to make own travel arrangements to patients' homes, clinics, base and meetings etc, as required
