An exciting opportunity has arisen in a community rehabilitation team for a band 6 physiotherapist to join a supportive and enthusiastic community therapy service as a Physiotherapist, working across community delivering rehabilitation in patients own homes following discharge from an acute bed, or supporting Long term condition management.

The team comprises of qualified physiotherapists, and assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in community.

You will be working with patients with a large variety of conditions and disabilities, in their own homes and local community. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team.

The service operates across 5 days , Monday to Friday

Your nominated base will be Blackburn with Darwen, however cross locality working across the East Lancs footprint at times.

### Main duties of the job

The post holder is an autonomous practitioner, responsible for provision of specialised assessment and treatment of patients referred to the service. . Responsible for managing a caseload of patients with a variety of complex conditions including long term condition management and acute hsopital discharges. You will see patients in own homes , working with an MDT in other organisations. You will carry out specialist assessment and treatment including the prescription of essential equipment as part of the therapeutic intervention .

You will play an important link with PCN and MDT teams in attending meetings as part of neighbour hood team working

The post holder is an autonomous practitioner, responsible for provision of specialised assessment, treatment, intervention and discharge planning of patients referred to the service. Responsible for managing a caseload of patients in community, and onward referral and discharge.

### Applicant requirements

You must have appropriate UK professional registration.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

**Essential**

- o Post graduate therapy experience to include community and rehab
- o Profession specific Diploma/degree in Occupational Therapy or Physiotherapy o Health Professions Council Registration o Evidence of CPD maintained in a portfolio including attendance at recent postgraduate courses relevant to the clinical fields in rotations. o Membership of the Chartered Society of Physiotherapy/ Royal College of Occupational Therapy o Involvement in Special Clinical Interest Group
- o Post graduate therapy experience to include community and rehab. o Post graduate training in relevant clinical areas o Experience of delivering training to colleagues in health and/or other agencies. o Experience of working in an integrated therapy or multi professional MDT o Experience of therapy student and/or support staff supervision o Contribution to clinical education of less experienced staff/students/assistants o Experience of working with patients with dementing
