You will work within CYP Urgent Care Services as part of an MDT of people managing our riskiest children and young people who are at risk of admission to hospital. Supporting the teams across the working week- Monday- Friday 9am-5pm, your base is negotiable and we do support some flexible working to enable a good work-life balance. ### Main duties of the job Devise, deliver and evaluate care relating to the physical health needs of patients via assessment (including risk), care plans, care pathways and service developments. Provide clinical leadership and work in partnership with patients, their carers and internal/external colleagues within the multi disciplinary (MDT) in the delivery of high quality, evidence based physical health care. Through education and training, support the development and drive improvements in physical health clinical practice skills with colleagues in local settings. Through liaison, joint working, training and education ensure that the clinical practice is in line with trust policy ensuring the trust objectives for improving physical heatlh care is achieved for our service users.
