### Job overview

As part of our strategic vision for improved physical health monitoring, care delivery and parity of esteem between physical and mental health; you will facilitate and support the management of the physical health needs of patients during their treatment and care within CYP services.

You will work within CYP Urgent Care Services as part of an MDT of people managing our riskiest children and young people who are at risk of admission to hospital. Supporting the teams across the working week- Monday- Friday 9am-5pm, your base is negotiable and we do support some flexible working to enable a good work-life balance.

### Main duties of the job

Devise, deliver and evaluate care relating to the physical health needs of patients via assessment (including risk), care plans, care pathways and service developments.

Provide clinical leadership and work in partnership with patients, their carers and internal/external colleagues within the multi disciplinary (MDT) in the delivery of high quality, evidence based physical health care.

Through education and training, support the development and drive improvements in physical health clinical practice skills with colleagues in local settings.

Through liaison, joint working, training and education ensure that the clinical practice is in line with trust policy ensuring the trust objectives for improving physical heatlh care is achieved for our service users.

To support the integration between CCETT, EDS and MHLS by sitting across all teams to ensure all patients have access to physical health support.

### Experience

- Previous experience of a work environment associated with challenging and complex situations
- Experience of communicating with a wide range of service users (patients), public, partners and staff
- Experience of managing the care of patients with complex physical healthcare needs
- 2 years post registration experience

### Qualifications

**Essential**

- Registered Health Practitioner - Registered Nurse (Adult, Child, Learning Disabilities and / or Mental Health); Occupational Therapist or Physiotherapist
- Where Learning Disabilities or Mental Health, a relevant qualification in physical healthcare or ability to evidence up-to-date knowledge and experience of working with physical healthcare needs in a mental health or learning disabilities setting
- Teaching qualification (ENB 998 or equivalent)
- Evidence of continued professional development

**Desirable**

- Experience of mental health or learning disabilities within a health or care setting
